Safety, Reliability and Risk Assessment
We can undertake qualitative and quantitative assessments of hazards and their associated risks to members of the public and workforce from new, modified or existing facilities and operations.
Assessments can be tailored to support the Customers management decision process or prepared as safety justification supporting Customer activities (for example, where a Safety Case may be required).
Methods and techniques include:
- Hazard identification - HAZID, HAZOP, FMEA
- Fault modelling - Logic trees , reliability assessment, availability assessment
- Consequence and frequency assessment
- Risk evaluation - Boston matrix, PSA, ALARP assessment
Human Reliability Assessment
We can undertake qualitative and quantitative assessment to determine the vulnerability of a system or operation to human error, often reported to be the cause of more than 80% of accidents and undesirable events.
Assessments may be used to identify contribution of human error to overall risk of operations and to identify measures and strategies to reduce the potential for human error.
Methods and techniques include:
- Human error identification - HAZOP, Task Analysis, SHERPA
- Human error assessment – HEART, THERP, NARA
Human Factors / Ergonomics
Human Factors and Ergonomics can be briefly described as the study or workplace environment (physical and organisational), and systems requiring operator intervention in order to improve the safety and efficiency of associated operations and the welfare of the operators involved.
Through appreciation of ergonomic principles, systems can be designed to meet both the physical and mental abilities and expectations of operators, ensuring increased operability, improved efficiency, reduced down-time, reduced stress levels and reduced absenteeism.

- Ergonomic Walkthrough / Walkdown
- Allocation of function (machine vs. operator control)
- Workspace / Layout Design
